• 論文抄録 / 内容記述: 人口 69 万人の A 市では，地域を基盤として医療を安定かつ効率的に統合的ケアとして提供できる仕組み作りが求められている．静岡県立大学では，大学 － 自治体 － 地域の連携による在宅医療 ・ 介護のリーダー人材育成支援プログラムを開発し，チームとしてのパフォーマンス向上を目的としたチーム STEPPS 研修を市内の日常生活圏域において地域包括ケアに携わる医療 ・ 介護の専門職者を対象に実施した．これに並行し，チーム STEPPS 研修に活用する教材の開発をすすめ，平成 27 年度には冊子教材，平成 28 年度は動画教材作成に着手した．さらに平成 28 年度には，医療 ・ 介護の専門職者を対象とした量的調査を実施し，チーム STEPPS 研修受講者における研修効果，地域包括ケアに携わる専門職の地域連携活動の実態を把握した．これら一連の取り組みについて報告する．

• 論文抄録(英) / 内容記述: City A with a population of 690,000 needs to develop a system of community-based healthcare that provides stable and efficient comprehensive care.In collaboration with universities, local governments, and communities, the University of Shizuoka developed a leadership development and support program for personnel working in home-based medical and nursing care. To enhance team performance, a Team Strategies and Tools to Enhance Performance and Patient Safety (Team STEPPS) training was conducted for medical and nursing care professionals that are involved in community-based integrated care within their spheres of daily life in the city. Concurrently, educational materials to be used for the Team STEPPS training were also developed. The development of an educational booklet and an educational video started in FY2015 and FY2016, respectively. Furthermore, a quantitative survey targeting medical and nursing care professionals was conducted in FY2016 to determine the training effect among participants of the Team STEPPS training and to understand the current level of collaborative activities in the community among professionals involved in providing community-based integrated care. This document reports on these initiatives.
