認知症のある本人のことを理解して支援を行うことは本人のエンパワメントに繋がり認知症の進行を遅らせることも明らかになっている．一方，本人のアセスメントを適切に行うための仕組みが確立していないため暗黙知として現場に留まっている．そこで本研究では，当事者視点の支援を実施するためのアセスメント方法として ICF を標準指標として採用した．本人の望みや個性を重視した当事者視点アセスメントやアセスメント情報を活用したケアプラン作成のためのデータの可視化・思考プロセスのアシスト機能を実装し，アセスメントからケアプランの作成・運用をサポートするシステムを開発した．実際の介護施設における実証評価実験を通して，システムがアセスメントの入力やケアプラン入力に有用であることが示唆された．また，システムユーザビリティの観点からは，システムからのサポート機能の充実化や長期的な運用が重要であることが明らかとなった． |

Individual care that understands and supports the person is effective in supporting the life of dementia. However, in most facilities, records, care plans, and care are not linked and the foundation for personalized care is not established. One of the reasons why individual care is not widespread is the delay in language standardization in the long-term care industry. Therefore, in this research, we built a system to create and operate a care plan using ICF as a standard index. The system supports assessment that emphasize the person's hopes and personalities by visualizing the data and expressing the thinking process. It was suggested that standardizing the data could create metrics, automate analysis and visualization, and support them. |
