Examples of such survey contain a survey to the university on recent educational achievement and a survey to the industry on the educational expectation to the academia. There are the cases when two or more interrelated questionnaires are executed simultaneously. In this paper, we propose a flexible questionnaire system for web-based survey on quality and quantity of educational outcomes. The core of the system is a web-based survey system “cresie” associated with a tool to support integration of the data at the user side and a tool to analyze the collected data. We develop a model of questionnaire and survey process so that the proposed system can be adapted to a wide range of web-based surveys related to quality and quantity of educational outcomes. Furthermore, the system supports the entire process of a web-based survey by providing the data analysis functions, utilization of Web-based and Excel-based answers and feedback functions to the responders.
