 note = {本稿では，石川県能美市における地域包括ケアシステム構築に向けた多職種連携のための会議と見守り活動に対して，ICT を活用した情報共有システム 「MCNBookSystem」 を用いて，現状と課題，影響を明らかにしていく．医師や介護士，見守り対象となる高齢者の家族も含めた多職種がこの MCNBookSystem を使用して，見守り活動の際に対象となる高齢者の体調や活動状況などを，スマートフォンやタブレットなどの携帯端末 ・ デスクトップ PC にて入力し情報共有を行う．その後のアンケート ・ インタビュー調査から，医師や介護士など，家族以外の各職種は，見守り活動において家族からの情報を重要視していることがわかった．しかし一方で，各職種間が持つ 「見守り」 の定義や目的に差異があることがわかった．, We introduce Memory Care Network Book System (MCN-Book System) for The Integrated Community Care System. Elderly care among multiple occupations in doctors, care givers, and families is considered important. Our system supports elderly care by sharing information among multiple occupations. Families, doctors and care givers used this system. We found the care effect of this system from questionnaire and interview. Based on the observation above, we consider the differences in the definitions and purposes of the Elderly Care between each job category.},
