• 論文抄録 / 内容記述: これまで日本では，国民皆保険制度の下，質の高い医療サービスが低コストで提供されてきたが，少子高齢化等に伴い，医療費は増加の一途 （平成 23 年度の国民医療費は 38.6 兆円） を辿っており，医療費が財政を圧迫する状況となっている．また，約 9 割の健康保険組合および約 9 割の地域中核病院が赤字となっているほか，地方や特定地域における専門医不足など，国民皆保険制度が破綻の危機に直面している．こうした日本の医療が直面している多くの課題を解決する－つの手段として，ICT を活用した医療の効率化が期待されている．本報告では，医療の効率化を実現し，医療健康分野における様々な課題を解決すべ< NTT の R&D において取り組んでいるシステム開発や事例を紹介する．具体的には，厚生労働省が推奨する地域包括ケアシステムの実現を目指して取り組んでいる，地域医療連携システム，遠隔医療・介護システム，医療・介護連携システム，および予防に向けた医療健康情報の活用等について説明するほか，自治体や病院と行った実証実験について説明する．

• 論文抄録(英) / 内容記述: In Japan, under the universal health insurance system, high-quality medical services have been provided at low cost. However, medical expenses has been increasing by the aging population, it is a situation in which medical expenses to financial pressure. (National health expenditure in 2012 was 38.6 trillion yen.) Moreover, regional core hospital of about 90% and health insurance association of about 90% is in the deficit, and specialist shortages in rural area, universal health insurance system is in danger of collapse. As a means for solving these problems, the efficiency of medical care using ICT are expected. In this report, we introduce the case and system developing in NTT R&D to achieve the efficiency of medical care in order to solve the various problems in the medical health field. Specifically, we describe regional medical cooperation system, telemedicine and telecare system, medical and nursing care coordination system, and use of medical and health information for the prevention is working with the aim to achieve area comprehensive care system that the Ministry of Health, Labour and Welfare recommended. Moreover, we describe field trials were carried out with the hospital and local governments.
